Have you verified this by pulling up the telemetry in test drive under tuning? Could be something to do with the controller itself. Contact Xbox directly and let em know what's going on and they'll probably send you a brand new controller for free. Neighbor across the street had issues with his mic and his kinect and they sent him brand new stuff for free (providing of course he sent back the busted stuff which they paid for the shipping on).
